Position Summary:
The RN Hospice Case Manager delivers nursing care to patients in their homes and coordinates their treatment plans with a supportive, multidisciplinary team. This role blends hands-on care with case management responsibilities in a rewarding, patient-centered environment.
Key Responsibilities
- Provide direct, skilled nursing care to patients in the home setting
- Conduct initial assessments and develop individualized care plans
- Evaluate and revise plans as needed, collaborating with team members
- Administer medications, perform wound care, IV therapy, and other clinical procedures
- Educate patients and caregivers on treatments, medications, and disease processes
- Ensure timely, accurate clinical documentation of care provided
- Verify patient eligibility and compliance with home health guidelines
- Active RN license in the state of Massachusetts
- Minimum 2 years of nursing experience
- At least 1 year of home health experience preferred
- Strong communication, assessment, and critical thinking skills
