About The Job
The Registered Nurse in our Family Practice provides professional nursing care to patients of all ages in collaboration with physicians, advanced practice providers, and the healthcare team. This role supports both routine and complex patient needs, delivering care in-person, by telephone triage, and through telehealth. Responsibilities include health assessments, patient education, chronic disease management, preventive screenings, medication administration, diagnostic testing, and coordination of care. The RN plays a key role in ensuring patients receive timely, compassionate, and high-quality primary care services.

About The Job
The Registered Nurse in our Family Practice provides professional nursing care to patients of all ages in collaboration with physicians, advanced practice providers, and the healthcare team. Responsibilities include health assessments, patient education, chronic disease management, preventive screenings, medication administration, diagnostic testing, and coordination of care. The RN plays a key role in ensuring patients receive timely, compassionate, and high-quality primary care services.
What You’ll Do
- Deliver the nursing process (assessment, planning, intervention, evaluation) across multiple care modalities.
- Collaborate with interdisciplinary teams to design and implement patient care plans.
- Administer prescribed medications, treatments, vaccines, and perform point-of-care testing.
- Educate patients on self-management, medications, nutrition, and wellness.
- Document care in the EMR with accuracy and detail.
- Provide clinical leadership, mentoring, and coaching to staff.
- Participate in ongoing education, skills training, and competency evaluations.
- Identify and respond to emergency situations according to facility protocols.
- Support practice initiatives, EPIC integration, and system-wide care improvements.
- Education: Associate of Science in Nursing required; BSN preferred.
- Licensure/Certification: Registered Nurse licensed in NH, an NLC state, or MA; current BLS/CPR required. Certification in Ambulatory Care Nursing or specialty area preferred.
- Experience: 1+ year professional nursing experience preferred; ambulatory care background a plus.
- Skills & Abilities: Strong clinical judgment, patient-centered approach, effective communication, time management, and adaptability in a fast-paced environment. Proficient in EMR systems and standard precautions.
